I have requested Christian prayer ministry from Servants of Jesus (SOJ) and its personnel (you) and I fully understand the nature of, the Christian prayer ministry that you offer.
I confirm you do not offer psychology, therapy or professional counselling and you don’t offer any health service and that I will consult a professional if I have any medical or psychological issues.
I acknowledge that I am not seeking medical, psychological, therapeutic and professional counselling from you.
I acknowledge that I am not asking you to try to change me and I only want you to provide non-directive prayer ministry.
I release you from all claims that I may have as a result of any of my confirmations being untrue or incorrect.
I agree to indemnify and to keep indemnified SOJ and/or its personnel, trustees, servants, representatives and agents, from and against any loss, claims, demands, disbursements, expenses, damages and costs suffered, incurred or brought against all or any of them, as a result of or arising from, the prayer ministry provided to me by SOJ.
I am aware that the under NSW State law the prayer team members are mandatory reporters regarding sexual abuse and may need to report anything of such nature.
